Initiative funded by the Paso Del Norte Health Foundation aimed to develop awareness about the importance of early detection and prevention of chronic disease in both urban and rural communities in El Paso and Cd. Juarez. Program strategies and activities focused on screening and outreach support to network providers in these communities through the use of local media, community presentations and a community newsletter featuring role models, outreach and education presentations to address early detection, screening, treatment and follow-up for chronic disease. The access to service delivery focused on wellness, addressing the importance of awareness of family risk factors, healthy lifestyle behavior change and annual screening and building on an existing provider resource network in both communities.
Partners: El Paso Diabetes Foundation, Fomento Social, and Paso Del Norte Foundation.Outcome: Educated 5,100 community members in our binational community about the importance of lifelong wellness emphasizing preventive care; coordinated a multidisciplinary network of wellness and prevention services for residents on both sides of the border; provided 6,870 screening and diagnostic services for diabetes, cardiovascular disease and cancer among residents of both El Paso and Cd. Juarez; and facilitated basic treatment and follow-up services to those residents with abnormal findings through the screening process.
